How do I provide a potential employer or educational institution a copy of my GED Test results? If requested for employment or educational reasons, original Official Reports of Scores or Official Transcript of GED Tests Results should be sent directly to employers or admissions officers. GED recipients must make a written request, either by mail or in person. In addition to the address where the score report should be mailed, requests must include full name, address, date of birth, social security/tax identification number, and signature. Sending scores by facsimile machine to any party or entity without the written consent of the GED recipient is prohibited. The DPE will only accept money orders or certified checks; however, walk-ins may pay with exact cash. It will cost:

• $10 for a duplicate GED Diploma or Certificate
• $10 for a duplicate GED transcript/verification
• $10 for a name change
• $10 for GED verifications
• $25 to convert a certificate to a diploma (to be eligible for conversion, the certificate must have been awarded on or after January 1, 2002--based on the results of the 2002 version of the GED Test)
